Sometimes called 'Manufacturer Certificate of Origin'.  This is the 'Birth Certificate' for a new efoil, ship, sailing vessel, motor yacht, personal watercraft etc.  It is a Department of Homeland Security U.S. Coast Guard form that shows the year built, hull identification number, place of build, and the dimensions. 

Note that efoils are NOT considered a personal watercraft (PWC).  They are a MONOHULL BARGE-SHAPE.  This makes sense when you look at the other choices of ship shapes.  It's not a V-hull, not a catamaran or trimaran, and it's not a sailboat with a keel.  It's just a very small flat-bottom barge.    

Personal Watercraft (PWC) is the term commonly used for jet skis, waverunners, Sea-doos etc.  In Florida, there are special rules and restrictions that apply to PWC above and beyond what applies to other vessels.  So It is good news that efoils DO NOT fit the definition of Personal Watercraft.  In Florida Statute 327.02 (37) Personal Watercraft are defined as 'vessels under 16 feet which use an inboard motor powering a water jet pump as the primary source of motive power'.  Therefore, efoils need to follow the rules for motorboats and other vessels, but not the ones specifically for PWC, such as the requirement to be at least 14 years old to operate a PWC.
